How Scoring Differs

Both tests produce a single number from 0 to 30, but the numbers are not interchangeable. The SLUMS uses education-adjusted score bands, so the exact points that flag possible mild neurocognitive disorder or dementia differ for someone who finished high school versus someone who did not (Shirley Ryan AbilityLab). The MMSE has traditionally treated 24 or above as within the normal range, though what counts as expected still depends heavily on age and education.

Because the two scales weight different tasks, the same person can score differently on each. A result near the top of the MMSE range can coexist with a lower SLUMS score precisely because the SLUMS asks more of memory and executive function. Which Test Clinicians Choose and When

There is no universal "better" screen; the right choice depends on the question a clinician is trying to answer. When the concern is subtle, early change — someone still functioning well but who has noticed a difference — the SLUMS may have an edge. In its original validation study of 702 patients at a VA geriatric center, the two tests detected dementia comparably, but the authors concluded the SLUMS was possibly better at detecting mild neurocognitive disorder that the MMSE failed to catch (Tariq et al., 2006).

The evidence is not one-sided, though. A later study that followed older adults over a year found the MMSE was, in some respects, more sensitive than the SLUMS to one-year changes that affected everyday functioning (Howland et al., 2016). Cost, familiarity, available translations, and whether a person already has a baseline on one scale all play a role too. In many cases a clinician simply uses whichever validated tool fits the setting and interprets it appropriately. What Happens Next

Whichever screen you take, a lower-than-expected score is a prompt, not a verdict. Neither the SLUMS nor the MMSE can diagnose dementia or Alzheimer's disease on its own. As the Alzheimer's Association explains, there is no single test that determines whether someone is living with Alzheimer's or another dementia — clinicians combine cognitive assessments with medical history, physical and neurological exams, and sometimes brain imaging or lab work (Alzheimer's Association).

When a result raises questions, a clinician may look for treatable contributors — such as thyroid problems, vitamin deficiencies, sleep issues, or medication side effects — and recommend further evaluation. The goal at every stage is clarity, not alarm.
